Has anyone ever had issue with being levied extra tax on
insured records? Bought a collection and was just wondering
if I should insure it or not?

In my experience, insurance values don't usually seem to have any influence on declared customs values. We regularly send postal and courier packages with higher insurance values and lower declared values and to date haven't had any issues with that.

Always seemed to me that the customs forms and the insurance forms are legal documents. If the numbers don't match...

You are bringing records into the US?
I don't know the numbers (but I am sure google does) but you can bring a high dollar amount of records into the US with out paying import tax.
The guys down the road, who sometimes buy containers full of records, have run into trouble from time to time.
If I recall they have had as more issues with the legality of importing records than with taxes.
Our friends the RIAA don't like furin records competing with Real American records.
