keepvid.com
or
vixy.net
In my opinion, Vixy has the best quality, but you have to do your downloading at weird times or else the server will always be too busy.
On the other hand, keepvid is always working and you can download youtubes with the quickness. I've made a couple mixed DVD's where I needed them fast and Keepvid worked like a charm.

Of course, you end up with an FLV file which can then be converted to MP4 with a program like iSquint (on OS X).
When you open your youtube video, go to Window->Activity Monitor (or press option+command+a).
You'll se a log-like window showing all activity in all currently open windows/tabs.
Find the activities for the youtube window, and look for the link that includes something like
cache.googlevideo.com/get_video in the URL.
That'll be the link to the embedded flash video file.
Double-click on that link and it will automatically start downloading in the download manager.
When the DL is done, you can play the resulting .flv file in Quicktime if you've installed the
Perian component (http://www.perian.org/), or convert it to the format of your choice with whatever
application you normally use for that. I think VLC can play .flv files too.
